To: All Principal Investigators and RCUH Employees
To assist in preparing for any potential operations changes, we have updated our RCUH FMLA Application form by adding a Part IV: Principal Investigator or Supervisor Acknowledgement. If an employee’s leave is foreseeable, the employee must notify and obtain a signature from his/her Principal Investigator or Supervisor prior to submitting their application form to RCUH.
As the policy states in section C) Responsibilities of Parties:
(1) Employee Must Request for Family Leave:
- If leave is foreseeable, the employee shall provide his/her direct supervisor and the RCUH Human Resources Department with advance notice by submitting a Family Leave Request Form at least thirty (30) days before the requested leave is set to begin.
- If leave is unforeseeable, the employee shall notify his/her direct supervisor and the RCUH Human Resources Department as soon as practicable.
