As part of our compliance with the Patient Protection and Affordable Care Act Law (PPACA), the RCUH is required to provide you with Form 1095-C. Form 1095-C will provide you with important information about the health insurance coverage that was offered to you (and your family if applicable) during the 2015 calendar year.
The REVISED distribution date for Form 1095-C: Friday, February 26, 2016
You will only receive a Form 1095-C if you:
(a) Worked 50% full-time equivalency or more, and enrolled in health coverage during the 2015 calendar year
(b) Worked 75% full-time equivalency or more, and were offered health coverage during the 2015 calendar year
All other employees who do not meet the above requirements will not be receiving a Form 1095-C.
For those that consented to receive an electronic Form 1095-C, you will receive an email notification when your electronic Form 1095-C is available.
