Advanced Payroll Deadline for Pay Period Ending 10/31/14 and other Advanced Deadlines

The payroll deadline for the 10/16 – 10/31/14 pay period is 12:00 noon on FRIDAY, OCTOBER 31, 2014.   The deadline has been moved up one day due to the General Election Day holiday.

 

Please note below the advanced payroll deadlines for the remainder of 2014.  All other payroll deadlines remain as usual (one working day after the end of the pay period).

RCUH – UNUM Group Long Term Care Initiative for Eligible Family Members

TO:               All Eligible Employees and Family members of The Research Corporation of the

                     University of Hawaii (RCUH)

SUBJECT:     RCUH – UNUM Group Long Term Care Initiative for Eligible Family Members

At RCUH, certain eligible employees are entitled to Long Term Care (LTC) benefits through RCUH’s group long term care policy with UNUM Life Insurance Company of America.  This notice is a reminder that eligible family members can also purchase a UNUM LTC policy under the group plan with the same discounted rates that are offered to employees.  This LTC policy is owned by the individual (not the RCUH) and will only be terminated upon request or due to non-payment of premium.

WHO IS ELIGIBLE

Immediate family members of the covered employee are eligible to enroll in a plan.  According to UNUM contract wording, “Family Members” are defined as:

·       Parents & In-Laws – the natural, adoptive or step-parents/grandparents of an Active Employee and their spouse;

·       Siblings – the natural, adoptive or step-siblings of an Active Employee and their spouse;

·       Children – the natural, adoptive or step-children of an Active Employee and their spouse;

·       Spouse – the legally married spouse of an Active Employee

Also, family members must be at least 18 years of age.

WHAT INFORMATION IS NEEDED

Each enrolling family member must submit a Family Benefit Election Form and Evidence of Insurability Form.  The Evidence of Insurability form is a short medical history questionnaire; it does not require a physical exam or physician statement.  The insurance company has the right to deny enrollment based on this information, however interested parties are encouraged to submit an application, as there is no application fee.

 

COVERAGE and COST

The cost of the monthly premium is determined by the coverage selected and age of member enrolling.  Example coverage options include:

·       100% Professional Home Care

·       100% Total Home Care

·       Compound Inflation (cost of living adjustment)

·       Monthly benefit amounts ranging anywhere from $1000/month – $6000/month

·       Benefit durations of 3 years, 6 years or Unlimited

Monthly premiums are billed directly from UNUM and paid directly to UNUM.  Spousal coverage may be payroll deducted from employee’s wages.

Safety Hazards for Hurricane/Tropical Storm Ana

SUBJECT:    Safety Hazards for Hurricane/Tropical Storm Ana

 

During a hurricane/tropical storm, employees may be exposed to additional safety hazards. It is important to be aware of these additional hazards as they may compromise workplace safety. OSHA has provided a list of various safety hazards that may occur during a hurricane/tropical storm. They are:

 

  • Hazardous driving conditions due to slippery roadways
  • Slips and falls due to slippery walkways
  • Falling objects (tree limbs and utility poles)
  • Flying objects (outdoor equipment)
  • Electrical hazards from downed power lines or downed objects in contact with power lines
  • Falls from heights
  • Burns/fires caused by energized line contact or equipment failure
  • Exhaustion from working extended shifts

 

Below are some general safety precautions that can be taken:

 

  • Hazardous Chemicals: Store all chemicals, fertilizers, and other toxic materials in a safe, secure area of the premises to avoid any chemical spills.
  • High Winds: Board up windows and glass doors to avoid shattered glass and close blinds/curtains. Be aware of any fall/trip hazards that may develop due to wind.
  • Outdoor Items: Secure any items that may be blown away or torn loose. If possible, secure them indoors to avoid any possibility of creating additional hazards.
  • Driving: Avoid driving is possible. If it is necessary to drive, do so with extreme caution.
  • Electrical Safety: Unplug all non-essential electrical appliances and avoid contact with any loose electrical wiring.
  • Communication: Prepare a contact list or phone tree to facilitate communications during severe weather.

2014 RCUH STATEWIDE UNITED WAY CAMPAIGN

 

 

Welcome to this year’s RCUH United Way Campaign!  During these tough economic times, we humbly ask for your continued support.  As one of the largest non-profit organizations in Hawaii, the Aloha United Way supports a network of member agencies to meet a broad range of health and human service needs.

 

 

The RCUH Statewide United Way campaign allows our employees to donate to the various island wide United Way programs:

  • Aloha United Way (Oahu)
  • Hawaii Island United Way
  • Maui United Way
  • Kauai United Way

 

HOW TO CONTRIBUTE:  To contribute to the various Statewide United Way programs, complete the 2014 RCUH Employee Pledge Form  and return the original form to RCUH Human Resources at John A. Burns, 4th Floor, 1601 East West Road, Honolulu, HI 96848.

Deadline: Wednesday, October 15, 2014. 

 

Contribute via Payroll Deduction:  RCUH employees have the option to contribute to the various Statewide United Way Campaigns via payroll deduction.  Your payroll deduction will be collectively sent to the United Way Campaign specified on your pledge form.  Payroll deduction period is from December 16, 2014 – December 15, 2015.

 

Optional Donor Choice Forms are available for you to contribute to a designated agency.  See the individual island campaign websites for more information and to obtain the Donor Choice Form.

 

 

For tax purposes:

 

Anyone who donates more than $250 will receive a tax acknowledgement letter from Aloha United Way in February.

 

A gift from $1 – $47.99 will receive a thank note via email.

 

A gift over $48.00 will receive a thank you note via mail.

 

If a payroll deduction pledge is made, please make a copy of your pledge form and use your last paystub of the year to show the charitable donation.
