RCUH Annual Performance Evaluation Forms have been revised for the July 1, 2016 through June 30, 2017 rating period. The forms are now accessible at our policy page. Please note that we have four options:
IMPORTANT NOTE: The deadline for receipt of performance evaluations is Friday, June 16, 2017 to be eligible for annual pay adjustments effective July 1, 2017. This deadline will not be extended; please submit forms early via any of the following:
