REQUEST FOR AN EIC DETERMINATION – NON-EMPLOYEE – GUEST SPEAKER
Instructions: Complete all Yellow boxes and check applicable boxes in (e) and (h). Email your EIC Memo/Attachment to RCUH Human Resources Department at least 7 working days prior to engagement of services ([email protected]).

(a) Method and Reason of Selection: Recognized expert in 4 (field of study/research). Invited to be guest speaker at 5 to be held at 6. This will provide our project with 7.

(d) Evaluation of results: No evaluation of the speaker.
(e) Duration of Action (Dates of engagement): Dates (from 9a – to 9b). This speaker engagement will occur as a one-time event as described on program outline.

(f) Method of Compensation: 10 honorarium (provide total dollar value, if exceeds $750 dollars provide justification).
