If you were previously employed with RCUH, please review these relevant topics below.

Former employees should retain access to their Employee Self-Service portal for 18 months after termination. If you need assistance logging in, please contact HR Help Desk at: [email protected] or (808) 956-8900.

 

Please ensure your contact information is current (mailing address and email address) so we can contact you with any relevant post-employment notices: COBRA continuation of coverage notice and enrollment forms, W-2 availability, 1095-C availability.

If you need a verification of former employment you may contact the RCUH Human Resources Department at (808) 956-3100 or via email at [email protected]. Faxes are also accepted at (808) 956-9423. RCUH Human Resources may require a signed consent form to release certain information to outside parties.

Please contact TIAA directly at 1-800-842-2776 or via their website at: tiaa.org for more information and to discuss your Retirement Plan options (e.g., withdrawal, annuitization, etc.) upon termination of employment. Cash withdrawals and rollovers may be subject to plan agreement limitations.

W-2 (Wage and Tax Statement): Upon hire, you were provided two options for W-2 delivery. You may make changes to your elections in Employee Self-Service:

1095-C (Employer-Provided Health Insurance Offer and Coverage):

RCUH can provide an IRS Form 1095-C to all employees who were deemed eligible for health insurance based on the regulations of the Affordable Care Act (ACA).  Beginning with calendar year 2025, you may request a copy via:

A Form 1095-C will be mailed within 30 days of your request to the mailing address on file in Employee Self-Service.

For former employees enrolled in an RCUH COBRA plan, you can expect new rates effective July 1st.  Please refer to the RCUH Employee Benefits COBRA page for rates.

COBRA Open Enrollment typically takes place in May (for a July 1 effective date) and during this time individuals currently enrolled in an RCUH COBRA plan may:

  • Change health insurance plans
  • Add qualified dependents

UH email addresses and usernames are governed by applicable UH policies and State laws, including UH Executive Policy E2.210 and Chapter 708, Hawaii Revised Statutes and are not actively managed by RCUH. RCUH employees are considered UH Affiliates and may have a grace period of up to 180 days of access to their email unless there is a request to terminate sooner. If you become re-employed by the RCUH as a regular hire having met the eligibility requirements for participating in the Group Retirement Annuity (GRA), you may be eligible to participate in the GRA immediately after re-employment provided eligibility requirements are met.

IMPORTANT: If you have made a withdrawal from your TIAA Group Retirement Annuity Plan (e.g., lump sum withdrawal, systematic withdrawal, etc.) AND have less than six (6) months break in service, you may not be eligible for rehire with RCUH. Please refer to Policy 3.210 Rehiring RCUH Retirees for more information.

If you were previously enrolled in a FSA Healthcare account and are rehired and meet re-eligibility within 30 days in the same plan year, this may reinstate your original election. If the break is longer, you may be allowed to enroll in a new election.

RCUH years of service, benefits (including medical, dental, retirement plans, Pre-tax accounts, and other insurances) vacation balance, and sick leave balances are independent from and not transferable to or from other state agencies (for example, University of Hawaii (UH) or Department of Land & Natural Resources (DLNR)).
